MEDICAL MANAGEMENT
SCOPE OF SERVICES
Medical Management emphasizes the integration of evidence-based practices, including Clinical Practice Guidelines ( CPGs) and focuses on holding healthcare systems accountable for optimizing patient outcomes. This allows us to improve patient outcomes through coordinated, integrated practices, incorporating the goals of case management, utilization management, disease management, and Exceptional Family Member Program. This also helps us provide more efficient approaches to healthcare delivery. Specifically, this allows us to shift from cost containment to continuous quality improvement taking into consideration the dynamic nature of Medical Management components within the Population Health continuum. We also recognize the importance of integration of communication between case management, utilization management, disease management, and exceptional family member program services for:
· Improving organizational efficiency & effectiveness
· Facilitating transitions of care & stewardship of resources
· Improving overall quality of care
· Decreasing duplication of effort & redundancy
· Decreasing negative outcomes from improper or poor handoffs
· Improving overall workload management

WHAT IS THE PATIENT CENTERED MEDICAL HOME (PCMH) PROGRAM?
- PCMH is a nationwide movement to improve healthcare
- Introduced in 1967 by American Academy of Pediatrics (AAP)
- Joint Principles of PCMH published March 2007
- Adopted by the Air Force in 2009
- Full Air Force Medical Service Implementation 2012
