Family medical, educational out-processing procedures clarified

3/13/2013 - KADENA AIR BASE, Japan -- One might ask, "What do I need to do to medically prepare my family to out-process?"

To help with the answer, the special needs coordinator offers the following information.

Once notified of his or her assignment, the Air Force active-duty member with dependents will need to complete Air Force Form 4380, Air Force Special Needs Screener, and take it to outbound assignments. The form can be obtained on the e-publishing website or out-processing link provided by 18th Force Support Squadron Outbound.

The special needs coordinator reviews the form for any family members identified with medical or educational needs and will contact the active-duty member. This form is completed whether the family member is on the island or not. The member should annotate on the form if the member's family is not on the island or will not travel to the gaining base.

The active-duty member with an overseas assignment or who is Q-coded with a stateside assignment should start the family member relocation clearance process six months prior to their date expected to return from overseas. The FMRC process starts at TRICARE with a brief from the FMRC coordinator.

The packages distributed should be completed and returned to the 18th MDG TRICARE section. The FMRC coordinator will contact the sponsor to arrange his or her FMRC screening. Screenings are conducted every Thursday from 9-11 a.m. at the 18th MDG Family Health Clinic.

The FMRC process may take up to 60 days to complete. Therefore, start early to avoid any delays. The FMRC coordinator will contact the sponsor to pick up his or her Air Force Form 1466, Request for Family Member's Medical and Education Clearance for Travel, when the process has been completed.
